To schedule a new evaluation, our office staff will need the following:

  • New patient forms filled out on website via a secure portal

  • Prescription/referral from physician for therapy

  • Insurance information (please include any secondary coverage, if any)
     

We accept a wide array of insurances including Aetna, Sunshine, Ambetter, Cigna, UHC, SimplyHealth, Optimum, Freedom, UMR, Tricare, Medicaid and Medicaid MMA's.

 

We are providers for the FES Scholarship (Previously Gardiner).

We are currently out of network for: Blue Cross and Blue Shield. 
 

We also offer Private Pay Packages.

What is needed for my first visit? 

We kindly ask you to fill out the Intake form prior to your appointment so that our staff may verify your eligibility and benefits to inform you of any possible copay/coinsurance, if applicable.

 

Please remember to bring a copy of your insurance card, photo ID, and applicable co-pay to the first visit, or send electronically to the office. All charges are due at the time of service. Acceptable forms of payments: cash, card, check, HSA cards,

Evaluations require a parent or caregiver present. A maximum of two adults are permitted. 

What to expect during my first visit?

During your first visit you and your child will meet with a certified therapist to discuss your concerns. Your therapist will then proceed with a variety of formal and informal assessments. When testing is completed a treatment plan may be developed to address your child’s individual needs. We encourage you to share your academic and developmental concerns at this time with your therapist. 

What to expect after my first visit?

Your therapist will write a formal evaluation report and treatment plan, based on your concerns and assessment results conducted during the first session. Your child’s plan of care will be sent to your physician and insurance for review prior to scheduling ongoing treatment services. Once your child is able to be scheduled for ongoing treatment, you will receive an email indicating that your insurance has approved the services. At this point, you can then book an appointment via our website and  the scheduling coordinator will contact you to set up a day and time that is convenient for you.

We encourage parents to be active participants throughout treatment, and welcome parents to join in sessions and join in play! Your therapist will make recommendations and give you home program strategies to support you and your child in achieving your goals!
